Notice

Website privacy notice

This notice explains what this website collects, why, and the choices you have. It covers the website only. Your dental records are covered by our Notice of Privacy Practices, which you receive as a patient. Ask our front desk for a copy.

When you request an appointment

The call-back request form asks for:

  • your first and last name and mobile number (required);
  • the reason for your visit, chosen from a short list (optional);
  • anything else you want us to know (optional). Please don’t include detailed medical history. We’ll go over it privately at your visit.

With your request, the website also sends the address of the page you sent it from and the last few buttons you clicked on this website during this visit (for example, “call” in the page header). These help us see how you found us.

Your request is emailed to the team that handles appointment requests for our office, so we can call you back and schedule your visit. Where our secure database is set up, a copy is also kept there, and only people who work on our behalf can access it. Your IP address is used briefly to stop repeated or automated submissions. It is not stored with your request.

The appointment form

What you type into the appointment form goes only to our office. Our website analytics does not read or record it, the form is left out of any visit recording, and we don’t send an “appointment requested” event to advertising companies. We count requests on our own systems.

Visit analytics

To understand how patients find and use this website, our website analytics service counts visits on every page: which pages you view, how you arrived (for example a search engine, another website or an ad link), your device and browser type, your approximate area based on your IP address, and how you scroll and click. It keeps a random visitor ID in your browser so repeat visits can be counted. It is run on our behalf and is not used to sell your information.

Cookies, recordings, advertising tools and your choices

Recording of your visit and advertising or marketing tools load only after you choose “Allow” in our cookie banner. If you choose “Decline”, or if your browser sends a Global Privacy Control or Do Not Track signal, they don’t load at all. Your choice is saved in your browser. To change it, clear this website’s data in your browser settings and we’ll ask again.

Counting button clicks

To learn which buttons help patients reach us, the website counts clicks on call, email, directions, appointment and before-and-after buttons. For each click it records only the kind of button, its words, where it sits on the page, and the page address. It sets no cookie, uses no visitor ID, and does not store your IP address. Clicks are not counted when your browser sends a Global Privacy Control or Do Not Track signal. Your browser keeps the last few clicks of this visit until you close the tab, so they can go with an appointment request as described above.

What we don’t do

  • We don’t sell your personal information.
  • We don’t send what you type into the appointment form to advertising or analytics companies.
